Intake Coordinator

Description:

Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.

We are currently looking for an Intake Coordinator to optimize our care services. The role is responsible for the admission process for new/returning patients across various Medi-cal programs.

Location: This is not a remote role and is located in our Chico office.

Schedule: This is a full-time role from Monday-Friday 8am to 5pm with an hour for lunch.

Travel: Some travel may be required.

Duties and Responsibilities:

  • Collaborate with clinical and management teams, call physician offices, verify eligibility of insurance and gain authorization for home health services and upload to patient's chart. Follow up authorization per organization timelines to ensure a timely admission
  • Receive referrals from physicians and facilities to initiate the intake process
  • Add new patients into the Electronic Medical Record (EMR) and enter all information required to create an episode, and track patient information
  • Coordinate with the Scheduling Coordinator to ensure all disciplines are staffed and assigned to each chart in the EMR
  • Receive medical documentation to scan into the system, following the company's scanning policies. Contact referral sources as needed to ensure all required documents are in the medical record
  • Coordinate with the clinical teams to ensure required documentation is obtained from physicians, hospitals and other referral sources
  • Attend regular interdisciplinary team meetings for purposes of identifying new admissions/referrals
  • Track recertifications via EMR reporting and validate ongoing insurance eligibility for new benefit periods
  • Review the monthly eligibility report and ensure that all insurance corrections are made to patient demographic information
  • Review, with clinical manager, assigned daily and weekly reports
Required Skills and Abilities:
  • High school diploma or equivalent
  • Knowledge of payer sources, insurance authorization requirements
  • Excellent phone and customer service skills
  • Skilled in operating a computer utilizing a variety of standard software, MS Office with Excel and Word, Outlook and electronic health records
  • Strong organizational skills and attention to detail. Medical knowledge acquired through experience or education
  • Experience in home health agency scheduling and accessing reports to update agency's clinical management
  • Respectful collaboration with all team members
  • Able to read, write and communicate in a professional manner.
  • Comfortable speaking via phone, with clinical staff and physicians in the community
  • Communicate timely updates to clinical management related to agency issues
  • A current California Driver's License
  • Current automobile insurance with agency specified limits and a reliable vehicle
  • Produce a Department of Motor Vehicles (DMV) printout prior to being hired that meets agency requirements
Preferred Skills: Experience with MatrixCare software or other home health electronic medical record systems

Physical Requirements:

Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirement for this role specifically include:
  • Sits, stands, bends, lifts, walks, and moves intermittently during working hours
  • Able to lift to 35 pounds without assistance
  • Able to drive intermittently

Compensation and Benefits:

Pay range is between $21-$25 per hour. Exact compensation may vary based on skills, experience, and location.

On the first of the month following 60 days of employment you will be eligible for health insurance, dental insurance, vision insurance, life insurance, as well as Flexible Spending Account (FSA).

Leave accrues starting on your first day of employment, and you can take paid vacation, sick and holiday leave after 90 days.

We offer a 401(k) matching up to 4% after 1 year of employment and 1,000 hours worked.
